Geriatric Resources, LLC offers:

Comprehensive Assessment:

An examination of the individual’s physical, emotional and psychosocial health, their living arrangements, social supports, legal concerns, advance directives, safety and the like.

Ongoing Care Management:

Assistance with selection of, implementation of, and monitoring of services within the home as well as accompaniment to medical appointments and reports of findings to designated family contacts.

Comprehensive Care Plan Development:

Establishment of and adjustments to an individualized plan of care specifying needs, frequency and duration of established services, in order to maximize the individual’s quality of life.

Phone Consultation:

Prior to the onset of services, Geriatric Resources, LLC provides a complimentary phone consultation, designed to identify problem areas and possible ways in which we can assist.

Home Safety Evaluations:

As part of the comprehensive assessment Geriatric Resources, LLC takes an in depth look at the physical environment of the individual and provides recommendations for improving the safety of the environment.

Resource Referral Services:

By evaluating the needs of the individual, Geriatric Resources, LLC is able to provide the individual with recommendations for needed services, based upon a thorough knowledge of the local healthcare industry and its offerings.
