Comprehensive Assessment

An examination of the individual’s physical, emotional and psychosocial health, their living arrangements, social supports, legal concerns, advance directives, safety and the like.


Ongoing Care Management

Assistance with selection of, implementation of, and monitoring of services within the home as well as accompaniment to medical appointments and reports of findings to designated family contacts.

Aging in Place

For many seniors, remaining at home is the most important need expressed when the realization occurs that they need greater support or assistance. While home to each of them means something different (their own home, an assisted living where they have been for years or that cozy independent living apartment) the value of remaining at home cannot be overstated.

By engaging the services of an Aging Life Care Professional (Geriatric Care Manager), the individual’s life in their home may be extended considerably. By assessing an individual’s needs and arranging for supportive & supplemental services to be brought into the home, seniors are afforded the ability to remain at home and their loved ones are afforded peace of mind.

Memory Care

Those who are experiencing a decline in memory often experience difficulties in recognizing areas in which they may benefit from additional support. While loved ones may see signs and identify changes, often the geographic distance between them prevents them from seeing the entire picture. Often, well-meaning/well intended neighbors and friends begin by providing some level of informal support, however, very quickly the responsibility becomes overwhelming.

An Aging Life Care Professional (Geriatric Care Manager) can assist by helping individuals and families to access the appropriate providers to determine the nature and scope of the individual’s cognitive loss as well as determining whether or not the observed symptoms are reversible or treatable. Additionally, an Aging Life Care Professional can direct the client and their loved ones to the appropriate services to help prepare for what may come in the future i.e. legal, financial, medical, and end of life services.

Geriatric Care Management / Aging Life Care Management

An Aging Life Care Professional, also known as a Geriatric Care Manager, is a health and human services specialist who acts as a guide and advocate for families who are caring for older relatives or disabled adults. The Aging Life Care Professional is educated and experienced in any of several fields related to aging life care / care management, including, but not limited to nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care.

The Aging Life Care Professional assists clients in attaining their maximum functional potential. The individual’s independence is encouraged, while safety and security concerns are also addressed. Aging Life Care Professionals are able to address a broad range of issues related to the well-being of their client. They also have extensive knowledge about the costs, quality, and availability of resources in their communities.
